Free spay/neuter is intended for free roaming community cats and personal cats in homes where paying for spay/neuter surgery would be a hardship to the family. You must fill out this form completely to be considered. Typing your name in the signature box indicates that you agree to all the terms and conditions.

Is this stray or feral cat that you feed? Are the caretaker of a colony of cats? Is this a cat that lives in your home? Do you need help trapping the cat or transporting the cat to the clinic?

Please read each item carefully and check the box indicating agreement.

I agree that if anyone is bitten by this cat during transport or at the veterinary clinic, the cat will be quarantined for 10 days.

I agree that any wound or injury found may be treated by the veterinarian including, but not limited to, wound care and antibiotic injection.

I agree that if this is a free roaming or community cat it will be ear tipped.

I recognize the risks cats face during transport, anesthesia, and surgery and agree to hold harmless SCRATCH, Inc., Board of Directors, Volunteers, Veterinarian and veterinary staff should a cat experience complications, injury, escape, or death.
